Don't have JEI? (aka Just Enough Items, previously NEI or TMI) Here's a list of crafting recipes for the summonors of all nineteen Bosses in the mod, as well as their specialized Bossic weapons:
Shrine of Air / Cannon of Air

Cannon of Air

Salt blocks are crafted with four salt items, in a 2x2 square. Salt can be obtained by slaying Mr. Derpington (summoned via the Summonor of Derpiness, crafted with an ink sac surrounded by eight of any raw fish) or mined from salt blocks within the ground.

Golden pillars are crafted with two gold blocks, one atop the other, producing two gold pillars.

Painstone crystals are mined from the bony landscape of the World of Agony. Agonized rations are crafted with three painstones on the top and bottom rows each, with the three slots in the middle row being filled by agonized melon slices, which are harvested from the agonized trees, also found in the World of Agony.

Gamma rays can be obtained simply by standing near a mutant villager for several seconds until he radiates on you (unless of course you are protected from such an event, as described on the main mod page). Gamma rays can also be made by smelting mutated blocks such as mutated planks. Infused iron is crafted with one iron ingot/block in the center slot, surrounded by eight uranium ingots/blocks (uranium ore can be found underground). The output is eight ingots/blocks of infused iron.

There are three varieties of pizzas that can be placed on the ground and then eaten, similar to cake. The default pizza (aka pepperoni) is crafted with three wheats on the bottom row, three milk buckets in the center, and in the top two corners, one steak and one cooked porkchop. Cheese pizzas are crafted the same, but without the meats. Pineapple pizzas are crafted by replacing the meats with pineapples, which are crafted by placing an apple in the crafting grid, with spruce leaves above it (or pine, if you will ^-^).

Terriblium brycium ingots are created by placing an iron ingot in the center slot of the crafting grid, with four grass blocks, one on each side, above, and beneath, producing four ingots. For blocks use the same amount of grass blocks, but with an iron block, to create four terriblium brycium blocks.

Ice crystals are made by placing a single block of packed ice in the crafting grid, with an output of four crystals.

Necromantic ores, from which are smelted ingots that can be made into blocks, are found in the Underworld. The gem of the dead, which is also required to ignite the portal to the Underworld, is crafted with an emerald in the center, with an ectoplasm on either side, above, and beneath. Ectoplasm is obtained by killing ghosts, which are found in some dark and/or cold biomes such as roofed forests and frozen oceans. Ectoplasm blocks, which are used to make the Underworld portal frame, are made with four ectoplasm each.

The Psi Sai can also be put in the crafting table with two feathers (shapeless recipe) to make a winged Psi Sai, that can be thrown. A winged Psi Sai can also be put in the table alone to revert back to the handheld variety.

Jade is found underground, and can also be made into blocks, stairs, etc. The religious tome, which gives the player added protection when in his/her inventory, is crafted by surrounding a book n' quill with four pieces of jade, one on either side as well as above and below.

This is the only Bossic summonor that is created using the Shards of another Element. That's just like Evil! to insist on wrecking the pattern for everyone else...
